2025 Season Waiver

ATHLETE WAIVER/RELEASE FORM ("Agreement")

IN CONSIDERATION OF being permitted to participate in any 2025 Inline Hockey Association of Arizona ("IHAAZ") activities ("Activities"), I, for myself, my personal representatives, assigns, heirs, and next of kin:

1. ACKNOWLEDGE, agree, and represent that I understand the nature of the Activities and that I am qualified, in good health, and in proper physical condition to participate in such Activities. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the Activities.

2. UNDERSTAND that:

     (a) Athletic activities involve risks and dangers of serious bodily injury, including permanent disability, paralysis, and death (hereinafter “Risks”);

     (b) these Risks and dangers may be caused by my own actions or inactions, the actions or inactions of others participating in the Activities, the condition in which the Activities take place, or the negligence of some/all of the Releasees named below;

     (c) there may be other Risks either not known to me or not readily foreseeable at this time, including, but not limited to, any exposure to the COVID-19 (SARS-COV2) novel coronavirus; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES that I may incur as a result of my participation or that of the minor in the Activities.

3. HEREBY RELEASE, DISCHARGE, AGREE TO HOLD HARMLESS, AND COVENANT NOT TO SUE the Inline Hockey Association of Arizona (IHAAZ), its member clubs, including its parent company and related affiliated and subsidiary companies of each, as well as the officers, directors, agents, employees, and assigns of each, and the IHAAZ's Associations, clubs, coaches, officials, administrators, members, volunteers, participants, sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activities take place, and any other party indemnified and held harmless by IHAAZ, (each considered one of the "Releases" herein) from all liability, claims, demands, losses, or damages caused or alleged to be caused by the negligence of the “Releasees” or otherwise, including negligent rescue operations, negligent security, travel, accidental transmission of the COVID-19 (SARS-COV2) coronavirus, and/or negligent recreational operations and activities.

4. Despite this Release and Waiver of Liability, Assumption of Risk, and Indemnity Agreement, if I, or anyone on my behalf, makes a claim against any of the Releasees, I will INDEMNIFY, SAVE, AND HOLD HARMLESS each of the Releasees from any litigation expenses, attorney fees, loss, liability, damage, or cost which any may incur as the result of such claim.

5. I have READ this Agreement and UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS by signing this.

6. I intend this to be a COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. If any portion of this agreement is held to be invalid, the balance, notwithstanding, shall remain valid.

7. MINOR RELEASE: As the minor’s parent and/or legal guardian, I understand the nature of athletic activities and the minor’s capabilities and believe the minor to be qualified, in good health, and in proper physical condition to participate in such Activities. I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS EACH OF THE RELEASEES FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON THE MINOR'S ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE, OR IN PART, BY PARTICIPATION IN THE IHAAZ, including the negligence of the “Releasees” or otherwise, including but not limited to, negligent rescue operations and accidental transmission of the novel COVID-19 (SARS-COV2) coronavirus. I further agree that if, despite this release, I, the minor, or anyone else on the minor’s behalf makes a claim against any of the Releasees named above,   I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE, OR COST ANY MAY INCUR AS THE RESULT OF ANY SUCH CLAIM.

Submit your WAIVER FORM FOR THE IHAAZ/Zona Cup 2025 EVENTS!

This online form will allow you to electronically read and sign the mandatory waiver and code of conduct for the IHAAZ 2025 season. Please make sure to carefully read all sections of this form, fill out your and your player's information, and electronically sign at the bottom.  

All players must complete this form before being added to a 2025 season IHAAZ/Zona Cup team roster. Players participating in multiple age divisions only need to complete this waiver one time. Please submit your waiver for your players' primary age group. Example: a 12u player is also playing up on a 14u team. That player only needs to submit a waiver one time for the 12u division team they are on.

Code of Conduct

• Sportsmanship is a key part of the game. No matter how hard fought or competitive a game is, when the final buzzer goes off and both teams have shaken hands, I understand I am expected to act professional on and off the rink.
• Game officials do their best to call a fair and impartial game. I might not always agree with every call but I will always treat the officials with the same respect they treat me with. If I have a question for the official, I will ask them politely during a stoppage of play for an explanation or have my coach or captain approach them on my behalf.
• I will treat every IHAAZ host facility like it was my own. I will not vandalize or in any way damage or deface any facility playing surface, locker room area, or other areas of the facility. I understand that if I do, I and my family will be responsible for all damages.
• I understand my actions on and off the rink have consequences, even if my coach or parents don’t find fault in those actions. I understand that not following RHA & IHAAZ rules may result in suspension of play and/or financial obligations.
• I have reviewed and am familiar with all RHA and IHAAZ rules and am committed to following them.

I, the parent or legal guardian for the player listed have read and reviewed with my player, the IHAAZ participant "Code of Conduct". The player agrees with the statements in the code of conduct and agrees to adhere to the rules and guidelines set forth in that code of conduct. The player and I, understand that the player may face additional consequences outside or in addition to the game officials penalties, suspensions, or other disciplinary actions that are assessed in certain situations.
